Prevalence of thrombocytopenia during pregnancy among libyan women

Thrombocytopenia is a recurrent hematological disturbance in pregnancy. In pregnant women with thrombocytopenia, the risk of excessive bleeding during or after child-birth is high, particularly when they require a cesarean section, other surgery during pregnancy, labor or in the puerperium period. The objective of this study was to assess the prevalence of thrombocytopenia among pregnant women attending antenatal care service at Tripoli University Hospital, Libya. A cross-sectional analysis was used to determine the prevalence thrombocytopenia in pregnant women attending prenatal care services from July to December, 2020 at Tripoli University Hospital. A structured pretested questionnaire was used to obtain socio-demographic data, nutritional factors, obstetrics and gynecological factors, history and clinical conditions. Venous blood samples were collected for platelet count and other platelet parameters, which determined by using Sysmex automation. A total of 72 participated women; about 40%, 35% and 259% of the participants were in the third, second and first trimester, respectively. The mean number of children that women have was 2, with 2.7% having just one child and 47% having three or more children. This study concluded that the prevalence of thrombocytopenia and mostly with mild form is 8.3% and as being greater among pregnant women living in rural areas. Health care providers should do thrombocytopenia test regularly for every pregnant woman in order to prevent excessive bleeding during pregnancy, especially whom living in rural areas.

A challenging presentation of pyrexia and macroscopic hematuria in a kidney transplant patient

Kidney transplant kidney transplant ( KTx) recipients have increased susceptibility to a spectrum of infections including; bacterial, viral, and fungal pathogens. Many factors contribute to their infection potential risk, in terms of their immunosuppressive state, that result in suppression of their B‑and T‑lymphocyte repertoire, also to consider certain donor and recipient‑related factors, that make them at risk of specific type of infectious complications. In addition kidney transplant patients tolerate poorly infections, which may adversely affect their graft function, by inducing glomerular injury, hence early diagnosis, directed therapy, and careful dosing of antimicrobial agents is of paramount importance in reducing patient’s morbidity and mortality. We report a kidney transplant patient, who received her live related kidney graft 7 years earlier (in 2013), she has been with stable graft function in most of her follow up period, until she presented with pyrexia, dyspnea and fatigue associated with deterioration of her graft function.

Simulation and experimental investigation on active solar coffee dryer

Abstract __ Considering the size and relevance of coffee industry to the Ethiopian economy this work presents the design, prototype development, analytical and numerical simulation as well as experimental investigation on an active solar coffee dryer. The solar collector and dryer system configuration has been optimized for minimal pressure drop by incorporating guide vanes and minimizing flow separation tendency using numerical simulation on ANSYS. The effect of air mass flow rate on optimal depth of collector, temperature rise and pressure drop were characterized including the effect of variation in solar insolation using CFD approach. By comparing different dryer configurations based on pressure drop and air flow uniformity, a new dryer configuration prototype was developed. In addition, thermal performance of the solar air heater was evaluated experimentally at three different airflow rates on a collector with corrugated absorber plate and another collector with flat absorber plate. High collector outlet temperature and efficiency were observed in a collector with corrugated absorber plate. The effect of depth of grain, moisture content on wet basis, airflow rate and humidity of air were explored for parametric sensitivity vis-à-vis drying time. Within 5 hours, coffee bean dried from 29% moisture content to 12.3% on a clear sunny day and within 7hours from 19% moisture content to 11.1% on a partially overcast day. Average thermal efficiency of the dryer was found to be 50.5% for clear sunshine day and 36.9% for partial overcast day. A good agreement has been observed between the experimental results and the CFD temperature rise predicted output with a deviation of 7.5%. Analytically predicted drying time compared with experimentally measured drying time within a 12.9% deviation. The cost -to-benefit analysis with a comparative assessment on traditional mode of coffee drying has been done and the payback period is found to be 1.03 years with significant benefits.

An empirical analysis of household consumption in rural maharashtra during march 2020

Current research investigates the nuanced impact of the COVID-19 pandemic on consumption patterns in rural Maharashtra, India. Our research has two main goals. First, we want to understand how factors like age, gender, job, education, and family size have affected how people in rural Maharashtra spent money during the pandemic. Second, we want to find out which of these factors had the biggest impact on spending in different parts of the state. Since rural areas have their own unique economic situations, we know it's important to look at how the pandemic affected these communities specifically. We're using math and tables to analyze the information and provide a detailed look at how the pandemic immediately changed spending habits and what it might mean for the future in rural households. This research is not only timely but also essential for informing policy decisions aimed at supporting rural communities in the aftermath of the pandemic. By unraveling the intricate dynamics of consumption in rural Maharashtra, we contribute to a deeper understanding of the economic challenges faced by these communities and provide insights that can guide targeted interventions to foster resilience and recovery.

Hierarchical temporal memory (htm)approach for fault detection in transmission line

This study was conducted to proposea hierarchical temporal memory (HTM) approach for fault detection in the Onitsha-Alaoji transmission line in Nigeria. Using a mixed research method, the study employed the Hawkins HTM model with two objectives and their corresponding research questions. The study gathered primary and secondary data to detect and evaluate faults in the Onitsha-Alaoji transmission line in Nigeria using HTM and compares its efficacy to current fault detection methods. With the use of simulation and descriptive methods of data analysis, results showed that partial discharge (PD) is the fault type that is being detected and it is commonly found as a fault leading to transmission line errors. More so, fault detection simulations were conducted at 40 km using typical power spectral density analysis. The first fundamental shifted from about 10 kHz to roughly 13 kHz during a fault. The HTM model outperformed sequence learning methods, resulting in a 90% mean test classification accuracy (CA) over extreme learning machine(ELM) and online sequential learning–extreme learning machine (OS-ELM), with OS-ELM performing poorly.The study concluded and recommended that the proposed HTM model be used to identify various PD fault types that plague the Onitsha-Alaoji transmission line in Nigeria. With the increased efficacy and reliability of the proposed model compared to existing methods, it is recommended for future implementation in this transmission line and potentially other fault-prone power transmission lines in Nigeria.

Relationship between culture and gender inequality in india

This paper throws light on the role played by culture and traditions specifically of Hindu religion in legitimising the subordinate position of women in Indian society. Along with presenting a brief account on the status of women from ancient times to contemporary situation, the sex-gender binary has also been explored. How a child after his birth socialised to behave in a certain way on the basis of his/her sex has been noted. This process of socialisation is based on the age old customs and traditions which are discriminatory in nature. The male child is taught to be strong, dominating and aggressive in nature and henceforth assigned laborious work to do for managing the finances of house. On the other hand, female child is taught to be sensitive, loving and caring and therefore assigned to manage household work, child nurturing and motherhood related responsibilities.

Traditional health practices concerning pregnancy, bırth, and the postpartum period of women giving birth in the hospital

Objective: This study was conducted to investigate traditional beliefs and practices of women regarding care of the mother and the infant during pregnancy, in childbirth, and in the postpartum period. Methods: This was a descriptive, cross-sectional study conducted at a public hospital in Istanbul. The data collected consisted of socio-demographic and obstetric characteristics, and responses to questions about some traditional customs regarding pregnancy, delivery, and the postpartum period. Results: In our research, some non-harmful cultural practices were found, such as the belief that to have a clever and beautiful baby the mother should eat fruit; that to have a healthy and peaceful pregnancy, the mother should not look upon ugly things; the mother should indulge her food cravings; and to have an easy birth, the mother should walk and focus on prayers. On the other hand, we also found beliefs that could be harmful, such as wiping the mouth of a baby with a date before breastfeeding, and practices believed to be protective that could cause harm, such as putting a knife under the baby’s bed, fastening a safety pin to the baby’s clothes, and for the mother and child to remain at home for 40 days. Conclusion: While non-harmful and beneficial practices related to maternal and infant health should be accepted and supported as a part of our cultural richness, practices that could be harmful should be prevented in pregnancy classes or with training upon hospital discharge

Water mass characteristic of ambon bay based on its temperature and chlorophyll-a distribution

Ambon Bay consists of the inner Ambon Bay (IAB) and the outer Ambon Bay (OAB) and they are separated by the shallow sill (12 m) Galala-Rumah Tiga. The topography of the bay makes it unique and causes the exchange of water mass between those two parts of the bay does not follow the daily tidal cycle. The aim of this study was to analyze the vertical and horizontal temperature and chlorophyll-a distribution in order to know the water circulation in Ambon Bay. This study was carried out during the monsoon, in November 2018. The CTD was used to measure in situ temperature and chlorophyll-a concentration. The data were analyzed by using Surfer and ODV programs. The results show that the temperature of the surface water (0-5 m) of Ambon Bay was 29.2℃. In depth 15-40 m, temperature was different between the IAB (27.7-28.2℃) and OAB (29,2-28.7℃). Chlorophyll-a concentration was found higher in the IAB than in OAB in all depths. In the depth >12 m, chlorophyll-a in the IAB was 1 mg/m3 compared to its concentration in OAB only 0.3 mg/m3. As a conclusion, the sill in Ambon bay causes unique circulation of water mass between IAB and OAB based on the analysis of temperature and chlorophyll-a concentration distribution.

Impact of preoperative baseline serum urea and creatinine levels among patients with perforated peptic ulcer disease in lafia, north central nigeria

Background: Perforation is the most serious surgical complication of peptic ulcer disease due to its attendant high morbidity and mortality. Hypovolemia, a consequence of gastro-duodenal perforation, is considered to be a cause of renal hypo perfusion with accompanied decreased clearance of urea and creatinine. The aim of this study was to determine the preoperative baseline serum urea and creatinine profile among patients with perforated peptic ulcer disease and the relationship of serum urea and creatinine with the outcome of care. Methods: A retrospective study of patients who had operation for perforated peptic ulcer disease over a 5-year period at Dalhatu Araf Specialist Hospital, lafia, North-central Nigeria was conducted. Results: Sixty-four patients were studied with a mean age of 36.6 ± 12.3 years. Majority of the patients were males, 59 (92.2%). While 38 (59.3%) patients had elevated preoperative baseline urea levels, 37 (57.8%) patients had elevated creatinine levels. Out of 29 patients who had surgical site infection, 16 (55.2%) had elevated serum urea (p = 0.535) while 19 (65.2 %) had elevated serum creatinine (p = 0.579). An overall in-hospital mortality rate of 14.1% was recorded. There was statistically significant relationship between elevated serum urea and inhospital mortality (p = 0.007). Conclusion: This result showed that preoperative baseline serum urea and creatinine levels were elevated in majority of the patients. In addition, the result of this study suggests that elevated baseline serum urea level has considerable relationship with in-hospital mortality

ROS (Reactive Oxygen Species) production via oxidative stress leads to selective neuronal degeneration and finally results in neurodegenerative diseases. PD (Parkinson’s disease) is one of the second most common neurodegenerative disease caused by overproduction of ROS due to the activity of xanthine oxidase. XO activity inhibitors may prove to be promising antiparkinson agents. Present investigation describes the synthesis and characterization of fluoro substituted pyrazolyl pyrazoline derivatives using spectrometer and 1HNMR spectra. Molecular docking studies of NAP1-NAP5 derivatives were performed in silico using molecular docking software. The resulting dock score in terms of D score has been compared with the standard XO inhibitor i.e. Allopurinol and Oxypurinol. The in silico approach used to study the Drug and Enzyme interaction using docking mechanism. Results indicated that the NAP derivatives accounted for better region specificity towards XO. It shows the strong binding interaction of ligand and receptor calculated on the basis of D score; hence the D score shows highest affinity. Overall score tabulation for D score as 2NAP1: -101.78; 2NAP2: -118.32; 2NAP3: -99.921; 2NAP4: -105.22; 2NAP5: -101.56; accordingly with the comparison of standard ligand molecule i.e. Allopurinol: -37.016 and Oxypurinol: -37.233.

Prevalence of vitamin d deficiency in medical students

The prevalence of vitamin D deficiency has recently been recognized in different parts of the world, even affecting healthy populations. The deficiency of vitamin D can lead to rickets in children and osteomalacia in adults. Few studies have been done to evaluate the status of vitamin D in the medical community around the world. No studies have been done in Libya to evaluate the status of vitamin D in medical students. The aim of this study was to evaluate the prevalence of low level of vitamin D in among healthy Libyan medical students of first year in University of Tripoli, Tripoli. Prospective study was conducted on 100 medical students of first year in Faculty of Medicine with a mean age of 19.5 years, 38 males and 62 females. Blood samples were taken and investigated for different biochemical parameters including serum calcium, serum vitamin D and serum parathyroid hormone levels. Questionnaire containing different data was completed for each student including personal data (age, address, nationality, and other data related to vitamin D deficiency. The prevalence of vitamin D deficiency in all the participated students was 74% (58.7% in males and 83.8% in females), while 21% had vitamin D insufficiency (28% in males and 16.12% in females). Only 5% of the students that had normal vitamin D was males. The mean 25-hydroxy vitamin D level was 19.49 ± 8.56 ng/ml in males and 12.08 ± 6.70 ng/ml in female students. Statistical analysis revealed that there is a highly significant difference among the student in vitamin D deficiency. In conclusion, low concentration of vitamin D is highly prevalent among the medical students included. An urgent action has to be taken in order to prevent adverse consequences of low vitamin D in young population.

Glp-1ra for glycaemic control and obesity as add-on therapy for type 2 diabetes

Diabetes mellitus (DM) is a complex and chronic illness requiring continuous medical care. Type 2 diabetes (T2D) is commonly associated with obesity, hypertension, and a tendency to develop thrombosis, and an increase risk of cardiovascular diseases (CVD). Diabesity is a term used to indicate the coexistence of obesity and DM. Diabesity increases as obesity is an emerging epidemic in modern societies, the co-incidence with DM is also rising, so a joint plan of anti-obesity and anti-hyperglycemia for the management approaches. Therefore, this study aimed to identify the impact of glucagon-like peptide-1 receptor agonists (GLP-1RAs) on body weight and glycemic response in obese Libyan patients with T2D at the National Diabetes Centre in Tripoli, between July 2013 and May 2022. This prospective study included obese adults with T2D who were newly prescribed GLP-1RA therapy for six months with dulaglutide once weekly or liraglutide once daily. The study included 170 diabetic patients who were started on GLP1-RA as add on therapy to their treatment, with a regular follow-up with dietitian and their physicians to adjust their glucose-lowering medications, then comparing the effect of these agents on body weight and the level of glycated hemoglobin before and after 24 weeks of treatment. Most of the patients (n = 99, 58.23%) were in the age period from 54 to 74 years old and 101 of whom were female subjects (59.4%), with a mean duration of DM equal to 8.8 ± 7.3 years. The patients were divided randomly into two groups, the first group included 110 patients who received liraglutide pens showed a significant reduction in HbA1c from 9.6% (± 1.54) to 7.4% (± 1.03) by p < 0.001 and a significant weight loss from 88.3 kg (± 10.68) to 80.8 kg (± 11.83) by p < 0.001. The reported adverse events were in 23 cases of minor hypoglycemia due to gastrointestinal upset. The other group included 60 patients for dulaglutide pens and showed significant decrease in HbA1c = 9.6% (± 1.54) to 7.1% (± 1.2) by p < 0.05 and a significant reduction of bodyweight from 88.3 kg (± 10.68) to 83.8 kg (± 16.3) by p < 0.05. The reported adverse events were mild transient gastrointestinal distress for the initial week of a start and then subside with regular intake. Whereas, 115 patients (67.6%) with HbA1c above 10.0% before starting therapy, no patient with HbA1c above 10.0% after six months of both GLP-RA agents therapy. Thus, the uses of GLP-RA as add-on therapy for obese patients with T2D significantly improved glycaemic control with less hypoglycaemia, accordingly, reduce insulin requirement for blood glucose control and loss in body weight. It can thus be concluded that GLP-1RA therapy is an effective treatment option when used in obese patients with DM.

Artificial neural network with crow search algorithm for optimal sizing of photovoltaic system

The need for renewable energy sources in addressing global energy demands is growing, especially in Nigeria where electricity demand often exceeds supply. Solar photovoltaic (PV) systems have become a viable solution, with federal universities in Nigeria, as major electricity consumers, recognizing their potential. However, determining the right size of PV systems for individual faculties within these universities is a complex task. This study attempted to simplify this process by introducing an innovative approach to size PV systems in these faculties. The research method used the Extended Kalman Artificial Neural Network (EKF-ANN) and the Crow Search Algorithm (CSA) to enhance the accuracy of PV system sizing. Data was collected on the study site, load demand, weather conditions, system components, and operational control and systems models to establish sizing criteria. The study focused on the optimal size of a solar PV system at the Faculty of Law building, University of Port-Harcourt, and how to improve its accuracy. The results showed that using global solar insolation parameters, EKF-ANN predicted values for global temperature, flock size, and maximal iteration. This optimized system could generate surplus power for effective grid supply. The study found that the optimal size of the series-connected panels for the Faculty of Law building was 96, 83, 73, and 65 units, with corresponding insolation values ranging from 3.737 to 4.368 kW/m2. It was concluded that the combination of CSA and EKF-ANN in solar PV sizing is suitable for achieving optimal outcomes for energy storage and grid supply. Nonetheless, the study recommended additional investigation into real-time and grid-connected solutions to enhance the proposed approach's effectiveness.

Analysis of perinatal mortality in aden general hospital: a hospital-based study from yemen

Background: Perinatal mortality remains high in developing countries, including Yemen. However, information on perinatal mortality in Yemen is lacking. This study aimed to find out the perinatal mortality rate in Aden General Hospital and to analyze the causes of perinatal mortality. Materials and Methods: This descriptive study was conducted at the Obstetrics and Gynecology Department of Aden General Hospital during the period from January 1, 2003 to June 30, 2004. It was a retrospective study for the first 6 months (January–June 2003) and prospective for the remaining period (July 2003–June 2004). Results: During the study period from January 1, 2003 to June 30, 2004, there were a total of 3770 births at Aden General Hospital, out of which there were 2976 births during the period from January to December 2003 and a further 794 births in the 6-month period from January to June 2004. The perinatal deaths were 166 (4.4%) giving a perinatal mortality rate of 44/1000 normal deliveries. Further analysis showed a total of 105 perinatal deaths in 2003 with a perinatal mortality rate of 35.3/1000 normal deliveries and 61 deaths in the period from January to June 2004 with a rate of 77/1000 normal deliveries. Unclassified deaths including premature cases accounted for 29% (n=48) of all causes, followed by mechanical causes 36 (21.7%). The mechanical causes are divided as follows, 33 (19. 9%) due to fetal distress and 3 (1.8%) due to breech presentation. Uncertain origin causes accounted for 19.3% (n=32) of all cases of death and toxemia was observed in 16 cases (9.6%), while malformation was found in 13 (7.8%) cases. Conclusion: The perinatal mortality rate in our center was 44/1000 deliveries. The major cause of perinatal mortality was prematurity, followed by mechanical causes. The most of the mortalities occurred outside the health facilities (extra-hospital).

Inflammation is a natural aspect of the immune system's response. Pain, redness, heat or warmth, and swelling are the four major symptoms of inflammation. Plants can generate a vast range of phytochemical substances as secondary metabolites, all of which have anti-inflammatory properties. Herbal medicines are significant cures for the treatment of a variety of disorders around the world. Medicinal plants are made up of roughly 7500 species, with representatives from about 17,000 higher flowering plant species. Even when synthetic chemistry has advanced beyond expectations, the utilization of natural products in the production of medications utilized in modern medicine is unrivalled. Anti-inflammatory medicines can help to reduce tissue damage and improve patient comfort by interfering with the pathophysiology of inflammation. As a result of the huge number of species available for investigation, the effective creation of new naturally occurring anti-inflammatory medications relies mostly on a multidisciplinary approach to finding new compounds. Despite the fact that many review articles have been written in this area, the vast majority of them only looked at the subject from a regional perspective. Various non-steroidal anti-inflammatory medicines have been proven to alleviate pain and inflammation by inhibiting the isoform of cyclooxygenase enzyme's digestion of arachidonic acid, hence lowering prostaglandin synthesis. Non-steroidal anti-inflammatory medicines come with a slew of negative side effects. Thus, there are, however, medicinal herbs that have anti-inflammatory therapeutic properties with a little or no negative side effects.

Screening of antimicrobial activity of murraya koenigii leaf extracts against pathogenic bacterial strains staphylococcus aureus and escherichia coli isolated from contaminated water

Aim: Murraya koenigii is a widely used plant both as a potential medicinal agent and also for common cooking purposes. Aim of this present study was to determine the antimicrobial activity of Murraya koenigii leaf extracts on Staphylococcus aureus and Escherichia coli. Study Design: Screening and isolation of pathogenic bacterial strains from contaminated water. Preparation of Murraya koenigii leaf extracts using petroleum ether, acetone and ethyl acetate by using serial extraction method with Soxhlet apparatus. Place and Duration of Study: Department of microbiology, Agro biotec research centre Ltd, Poovanthuruthu, Kottayam, Kerala, India, between 2014 January to 2014 May. Methodology: Staphylococcus aureus and Escherichia coli were the bacterial strains used in this study. Morphological and biochemical analysis of microorganisms were conducted to identify the strains. Leaf extracts (petroleum ether, acetone and ethyl acetate) of Murraya koenigii were screened using MHA disc diffusion methods. Results: Various concentration of plant extracts were used to check its activity against isolated pathogens. Acetone extract of curry leaves exhibit maximum zone of inhibition against Staphylococcus aureus and petroleum ether extracts shown maximum inhibition against Escherichia coli.

New mathematical model for software quality prediction of component-based software using shuffled frog-leaping algorithm

Customer satisfaction and profit making are the two motives that define software quality; therefore, software industry uses new technologies like component-based software engineering, re-engineering, etc., to make their software production more profitable. The proposed mathematical model is executed under ISO/IEC 9126 quality assurance model and justifies the definition of software quality given by IEEE 1061(1998). The model calculates the degree of stakeholder satisfaction (Q) by combining the quality attributes and it is validated using Shuffled Frog-Leaping Algorithm (SFLA) which improved the result by 2.46%
